Community Health Needs Assessments. Community Health Solutions has conducted community health needs assessments (CHNA) for multiple hospitals, health systems, and foundations. Most recently, we have helped more than two dozen not-for-profit hospitals complete CHNAs as required by the Patient Protection and Affordable Care Act.

Community Health Data Portals. Community Health Solutions is the creator and producer of the Virginia Atlas of Community Health. With our partners at GeoHealth Innovations, we have developed a leading edge portal for interactive mapping, data analysis, and action planning. Hundreds of organizations use the analytics available from the Atlas to inform their community health improvement strategies.

Community Health Improvement Initiatives. Community Health Solutions has provided support for many different community health improvement initiatives on topics such as healthy weight promotion, children's behavioral health, chronic care improvement, patient centered medical home, and more.
