Community Health Solutions is providing support for two collaboratives aimed at helping primary care providers develop the capacity to become accredited as patient centered medical homes. We are providing analytic support, strategy support, and online support for knowledge management, innovation, and collaboration.

Chronic Care Improvement. Community Health Solutions is providing support for multiple collaboratives aimed at helping primary care practices improve their quality of care for patients with chronic conditions. We are providng analytic support, strategy support, and capacity building support including an online platform for quality reporting and knowledge exchange.

Early Childhood Community Health Solutions is providing support for an early childhood developmental screening project operated by a state public health agency in collaboration with a statewide primary care association. The project provides training, coaching, and reporting support to help multiple physician practices improve early childhood developmental screening and referral rates. Community Health Solutions provides analytic support and capacity building support including an online platform for quality reporting.
