Community Health Solutions
- Helped more than two-dozen hospitals conduct community health needs assessments;
- Helped scores of primary care practices teams (including many safety net practices) build capacity for patient-centered care, especially for patients with chronic conditions;
- Helped dozens of community-based organizations define and communicate the value they deliver to the communities they serve;
- Helped multiple health grant makers develop effective strategic initiatives for community capacity building and health improvement;
- Supported the design and development of the Virginia Center for Health Innovation, a ground-breaking organization dedicated to accelerating the adoption of value-driven models of wellness and health care throughout Virginia;
- Co-developed the Virginia Atlas of Community Health, an innovative, online source of data and tools for community health improvement; and
- Supported the design and implementation of multiple leadership institutes for health professionals including physicians, nurses, and nonprofit executives.
