Community Health Solutions is supporting groups of federally qualified health centers and free clinics in their efforts to achieve recognition as patient centered medical homes. Community Health Solutions is providing strategy support, analytic support, and capacity building support including onsite learning sessions, technical assistance, and an online platform for quality reporting and knowledge exchange.

Chronic Care Improvement. Community Health Solutions is supporting safety net providers including primary care providers and behavioral health providers in collaborative efforts to strengthen chronic care for uninsured patients. Community Health Solutions is providing strategy support, analytic support, and capacity building support including onsite learning sessions, technical assistance, and an online platform for quality reporting and knowledge exchange.

Free Clinic Strategy. Community Health Solutions is supporting a statewide collaborative designed to help dozens of free clinics develop strategies for health care reform. Community Health Solutions is providing strategy support, analytic support, and capacity building support including onsite learning sessions, technical assistance, and an online platform for learning and knowledge exchange.

Behavioral Health Innovation. Community Health Solutions is supporting a statewide collaborative of community behavioral health providers as they build capacity to demonstrate performance in an accountable care environment. Community Health Solutions collaborated with participants to develop an innovative community return on investment model for behavioral health services, along with an analytic framework for performance measurement in an accountable care environment. Community Health Solutions is supporting implementation of these tools with onsite training, technical assistance, and an online platform for learning and knowledge exchange.
