Smart Collaboration for Learning and Capacity Building

A learning collaborative is a planned process in which multiple people collaborate to build their performance capacity through shared training and technical assistance. A learning collaborative could be created to support a community, a group of organizations, or a single organization. We have supported learning collaboratives for organizations aiming to build their capacity in asthma care, cardiovascular disease care, COPD care, chronic care generally, community return on investment analysis, early childhood screening, patient centered medical home, and program evaluation. We have also supported learning collaboratives on leadership development for physicians, nurses, and nonprofit executives. The typical learning collaborative includes a data portal and a knowledge portal in addition to group learning sessions and individualized technical assistance.

Smart Collaboration for Population Health Improvement

A population health collaborative is a group of organizations working together to achieve collective impact for a defined population. The population may include residents of a community, members of a patient population, a group of employees, or any other defined group of people. The work of the collaborative is focused on working together to improve the health of the population. We typically support a population health collaborative with data, best practice research, practice tools, training, and technical assistance. All of this content is housed in a secure online portal where people can view the results and contribute to the process.
